Position Overview:

As a Licensed Practical Nurse (LPN) with Ascend Nurses, you will play a pivotal role in patient care, ensuring each individual receives the attention and medical care they deserve. Key Responsibilities:

Conduct initial patient assessments to gather information on health conditions and medical histories Administer medications and treatments under the guidance of registered nurses or physicians Monitor patient health and record vital signs, promptly reporting any concerning changes Collaborate with a multidisciplinary team to support the implementation of individualized care plans Educate patients and their families on health conditions, medications, and post-discharge care Ensure all nursing activities adhere to established protocols and regulatory guidelines Engage in continuous learning opportunities to stay abreast of the latest LPN practices.

Qualifications:
