Medicare Wellness RN - Remote | WFH - #1339030

Position Overview

We are seeking a compassionate and skilled Medicare Wellness Registered Nurse (RN) to conduct Medicare Wellness visits with our patients, either in person or via telehealth. In this role, you'll collaborate closely with our providers to ensure thorough documentation and review of patient care, demonstrating excellent clinical judgment and active communication throughout.

Key Responsibilities

Conduct Medicare Wellness visits with patients, utilizing both in-person and telehealth platforms.

Collaborate effectively with providers to ensure comprehensive patient care and documentation.

Educate patients on chronic disease management and preventive care, offering support and guidance as needed.

Conduct pre-visit chart reviews and coordinate outreach efforts for preventative services and Medicare Annual Wellness visits.

Perform administrative tasks such as scheduling, verification activities, and patient communication via phone or other channels.

Assist with triaging chronic care management and remote patient monitoring as required.

Additional duties as assigned to support the healthcare team.

Qualifications

Current RN license in [State].

Minimum of two years' experience in primary healthcare or a related setting.

CPR certification required.

Work Environment

Regular interaction with diverse stakeholders, including team members, providers, patients, and insurance companies.

Flexible work hours to accommodate office schedules and ensure coverage during operational hours.

Participation in staff meetings and quality improvement projects aimed at enhancing patient care.

Occasional travel may be required to attend meetings or trainings.
