Expected pay for this position is $28.37-$33.38/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
Overview
POSITION SUMMARY:
The Hospice Liaison is a versatile representative of the hospice agency. The liaison acts as a key resource and supports access to care by meeting with patients and families and is responsible for identifying, developing, and retaining internal and external referral sources, ensuring high value customer service. The Liaison possesses strong communication and interpersonal skills to compassionately coordinate hospice admission and ensure smooth care transitions. The position requires a motivated, independent professional with demonstrated ability to identify opportunities, develop and execute tactics in alignment with strategic plan, expanding the health care community's knowledge and referral activity to achieve goals.
Qualifications
REQUIRED QUALIFICATIONS:
Education: Bachelor’s degree
Experience: 2 years of healthcare, social service, marketing or sales experience
Licensure/ Certification: N/A
Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
Preferred Qualifications
Education: Bachelor's in healthcare related field, counseling or business administration Experience: Hospice/Home Care experience. Licensure/ Certification: N/A Other Skills/ Knowledge: N/A
