As our Intake Coordinator, you'll be responsible for managing the full patient intake process—from the first referral call to scheduling in-home care. You’ll ensure a smooth, compassionate transition for patients entering hospice care, while working collaboratively with families, referral sources, and interdisciplinary teams.

This includes establishing and maintaining positive relationships, responding to customer requests and concerns, and managing the insurance verification, authorization processes and scheduling of HHA staff. The intake coordinator will also establish effective communication and collaboration with all levels of administration, customers, referral sources, and IDG team.

Essential Functions/Duties
- Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
- Ensures compliance with all state, federal, and CGS referral/intake regulatory requirements.
- Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients when navigating to different levels of care. Providing direct oversight and collaboration with all parties.
- Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
- Builds and monitors community and customer perceptions of Aspire for Well-Being Hospice at The Highlands at Wyomissing as a high-quality provider of services.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Offers insight into strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs.
- Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract provisions.
- Capability of effective planning and priority setting. Ability to meet deadlines.
- Strong attention to detail
- Participates in quality assessment performance improvement teams and activities.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Aspire for Well-Being Hospice
- Maintains confidentiality of information without exception.
- Demonstrates creativity in coordinating departmental staff schedules.
- Basic knowledge of hospice budget as it relates to overtime and staffing ratios.
