Shift: 10:00am-6:30pm

Location: Travel within Seminole, Orange and Osceola Counties.

The Role You’ll Contribute

The Hospice Program Representative facilitates the registration of patients to the hospice service in the hospital. The Hospice Program Representative is responsible for obtaining signed consents for the hospice program through meeting with/providing hospice solutions to prospective patients and families. The Hospice Program Representative will document the encounter and outcomes in the patient's chart utilizing the electronic medical record.

The Value You’ll Bring To The Team

  • Participates in the explanation of Hospice service and benefits to patients and families in order to facilitate the admissions process. (65%) * Travels to patient’s current location (hospitals, skilled nursing facilities and assisted living facilities) to facilitate initiation to hospice service * Identifies objections, concerns or other entry barriers and provides information in response to address those issues. * Describes and explains services to potential patient/family and elicits responses to questions regarding patient’s fears, physical limitations and attitude toward hospice care in order to put the patient and family at ease about choosing hospice and to gather pertinent information necessary for admission. * Speaks with patient/family regarding financial obligations and insurance coverage, using knowledge of the Medicare Benefit as well as other Medicare and other insurance reimbursement. * Obtains necessary patient consent and pertinent admission forms such as the Medicare Election of Benefit. * Enters encounter information into Electronic Medical Record and completes necessary documentation. * Communicate immediate care needs to hospice referral/admissions manager. * Coordinates with hospice Referral Center and/or Hospice Admission Nurse or Hospice Specialist for timely admission following patient discharge from hospital or facility. * Coordinates with the admission nurse regarding the admission. * Accepts assignments as designated from referral center staff and Referrals/Admissions Manager.
  • Quality Management (includes documentation and regulatory standards) (10%) * Thoroughly complete required documentation * Provides accurate information related to insurance, medical group and demographics * Develop and recommend policy and procedure changes as needed * Inform manager of quality issues * Collaborates with coworkers on staffing schedules
  • Fiscal Responsibility (10%) * Submits documentation in a timely manner and in compliance with relevant agency policies and procedures * Accepts assignments as designated by the supervisor * Practices effective time management techniques to meet productivity standards * Maintains positive professional relations with support personnel through appropriate and timely responses for information or paperwork necessary for the key clinical and billing functions of the agency * Must remain current with all company communications
  • Additional Duties and Responsibilities (10%) * Participates in staff meetings, department meetings, in services and other related activities. * Attends educational and developmental seminars and meetings, as deemed appropriate. * Recommends strategic plans on department goal setting for upcoming year(s) to manager. * Maintains membership of and participates in applicable hospice-related and professional organizations.

Minimum Qualifications

    • The expertise and experiences you’ll need to succeed:
    • Associate Degree
    • An equivalent combination of education and experience may be substituted at the discretion of the Administrator.
    • Customer service background. Strong sales skills with ability to assist families in choosing the hospice options.
Preferred Qualifications

  • Bachelor's in social work
  • Healthcare experience
