Position Status: Full-Time (1.0 FTE)

Schedule: Monday – Friday, 8:00 AM – 4:30 PM; no weekends, no holidays

Position Overview:

The Clinical Nurse – Ambulatory Navigator/Triage Hem/Onc is responsible for coordinating care needs for oncology and hematology patients by providing triage symptom management over the phone and ensuring accurate documentation of patient needs. The incumbent will collaborate with patients, their primary physicians, and a multidisciplinary team to develop customized care plans, manage treatment delivery, and advocate for patients throughout their healthcare journey.

Key Responsibilities:

  • Coordinate the care needs of assigned oncology/hematology patients and develop comprehensive plans to manage care delivery across the patient care continuum.
  • Triage symptom management and offer clinical advice over the phone based on patients' individual needs.
  • Collaborate with physicians, nurses, health professionals, social workers, and others to ensure appropriate tests and treatments are delivered in a timely fashion.
  • Advocate for patients to enhance their treatment experience and address their needs effectively.
  • Balance care needs with financial considerations to achieve efficient and effective patient outcomes.
  • Maintain thorough documentation of patient interactions, care plans, and triage assessments.
  • Participate in quality improvement initiatives within the department to enhance care delivery processes.


Qualifications:

  • Associate of Nursing (ASN) required; Bachelor of Nursing (BSN) preferred.
  • Active Registered Nurse (RN) license in the state of Indiana or active Nurse Licensure Compact (NLC) RN license required.
  • Must have graduated from a nationally accredited nursing program.
  • 3-5 years of relevant nursing experience preferred, with a focus on oncology/hematology experience preferred but not required.
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) required; other advanced life support certifications may be required per unit/department specialty.
  • Specialty certifications may be required in accordance with the department's provision of care.
  • Knowledge of personal computers and proficiency in various types of word processing and database software required.
  • Requires clinical work experience, understanding of medical terminology, and familiarity with medical record requirements, regulations, and policies.
