Example Of Duties
- Provide brief clinical support and psychoeducation to birth, foster, post adoption and other identified caregivers by offering targeted sessions that address immediate concerns and equip them with tools to manage youth behavior effectively.
- Assist in maintaining youth stability within their community placements by identifying early signs of distress and intervening with strategies to mitigate potential disruptions. (in person, telecommunication, or phone support)
- Knowledge of community resources to suggest services in home, office, or virtual settings to maximize accessibility and accommodate the diverse needs of families. (in tandem with regional Case Manager and Intensive Care Coordinator).
- Deliver clinical support and psychoeducation to birth, foster, post adoption and other identified caregivers during youth transitions from congregate care by ensuring they understand behavioral expectations and best practices for a smooth reintegration.
- Focus on stabilizing youth post-discharge to prevent re-entry into congregate care through structured check-ins and reinforcement of coping mechanisms.
- Coordinate with the discharging treatment team to ensure a “soft” handoff and continuity of care by facilitating a collaborative transition plan that aligns with the youth’s therapeutic goals. (working together with DHW Case Manager to coordinate and collaborate for youth/family needs).
- Dependent upon age, meet with youth to assess treatment readiness and overall treatment needs through structured assessments and goal-setting discussions.
- Provide short-term clinical intervention to support treatment planning by reinforcing therapeutic strategies and addressing immediate emotional or behavioral concerns.
- Knowledge of service availability per community which can be accessible in multiple formats: in-home, in-office, and virtually, ensuring flexibility and responsiveness to family needs.
- Families engaged in services will have access to 24 hour helpline for crisis support, offering immediate guidance and intervention when necessary.
- FSC services will be reviewed by a supervisor every three weeks to assess progress and determine next steps, ensuring accountability and continuous improvement.
- Documentation of all interactions, support provided, and coordination efforts will be maintained to ensure continuity and effectiveness of care, utilizing standardized tracking systems and case notes.
- FSCs will provide timely updates to the regional Case Manager who submitted the request for support through structured reports and scheduled check-ins.
- Regular communication will include the initial engagement of services, progress updates, and confirmation of transition to long-term community-based support when applicable, ensuring transparency and collaboration.
