- Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge
- Creates and maintains up to date aftercare coordination community resources
- Displays active involvement in treatment planning process, by setting up the aftercare appointments as identified by the patients’ provider and therapist.
- Provides resource information to patients and families as appropriate for future care needs.
- Actively communicates with clients, families, aftercare providers, and referral sources. Demonstrates proactive communication with those involved with the patients’ treatment.
- Bachelor’s degree in social work, counseling, psychology, or relevant state licensure or at least three years of clinical experience within an inpatient behavioral health setting.
- Prior experience with psychiatric and chemical dependency patients.
- Unencumbered clinical license if eligible/needed per state regulations preferred.
- CPR certification and Crisis Prevention Training (CPI) within 30 days.
