Job Summary

The Bereavement Coordinator coordinates the development and implementation of a plan of care as desired by the client and family. The Bereavement Coordinator completes bi-psycho-social assessments and provides appropriate social work and care coordination interventions. Serves as a resource to the community for bereavement issues. Performs other related duties as assigned.

General Requirements

  • Adheres to the hospital and departmental attendance and punctuality guidelines
  • Performs all job responsibilities in alignment with the core values, mission and vision of the organization
  • Performs other duties as required and completes all job functions as per departmental policies and procedures
  • Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs)
  • Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.
  • Must have a valid driver's license and reliable transportation for travel to patient's home.

WORKING CONDITIONS

  • General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
  • May be exposed to high noise levels and bright lights.
  • May be exposed to limited hazardous substances or body fluids, or infectious organisms.
  • May be required to change from one task to another of different nature without loss of efficiency or composure.
  • Periods of high stress and fluctuating workloads may occur.

Education Requirements

  • Bachelor's Degree in Social Work (Required) OR
  • Associate's Degree and 4 years of experience utilizing a comprehensive and tailored care plan that provides innovative solutions, dedicating sufficient time to patients to establish their holistic care objectives, and collaborating with physicians, nurses, therapists, social workers, and other members of the patient care team to implement and comply with a customized care and treatment regimen. (Required)

Experience Requirements

  • 1 - 2 years of Experience working with individuals who have experienced loss (Preferred)
  • 1 - 2 years of Experience leading support groups (Preferred)
