Shadelands Post Acute Rehabilitation Center is seeking a dedicated and passionate Physical Therapist Assistant to join our team on a full-time basis. Our facility focuses on delivering the highest quality care to our patients, with a current patient base that includes 59 Medicare patients and no long-term care residents.

Key Responsibilities

• Treat patients as directed by Physical Therapist.
• Record daily treatment notes and weekly progress notes per PT Board.
• Assist in maintaining department. Participate in Patient Care and Rehabilitation Conferences, as needed.
• Assist with cleaning and maintenance of treatment area.
• Treat patients per the physician treatment plan.
• Assist nursing department with training of Restorative Aides.
• Communicate with supervisor and other health team members regarding patient progress, problem and plans.
• Participate in in services training program for other staff in the facility. Record treatment changes per policy and procedures.
  
  

Qualifications

• Active physical therapist assistant license
