DUTIES

Essential Duties

  • Integrates into school and community settings to assess student and family needs related to all social determinants of health (such as housing, food, and transportation) and connects participants to resources identified via needs assessments.
  • Conducts outreach to identify community members who may benefit from health education and support services, linking them to appropriate care within our agency and with external partners. Outreach includes phone calls, in-person visits, letters, emails and text messages.
  • Assists participants in enrolling or maintaining enrollment in Medi-Cal or other assistance programs to improve their health, as pursuant to a plan of care.
  • Attends assigned school and community events and meetings on behalf of CII to represent the organization and support program visibility and enrollment.
  • Serves as a liaison between participants, resource teams, and other community resources.
  • Establishes and maintains rapport and effective working relationships with children and families, collaborating within CII and the broader community.
  • Initiates and maintains communication flow between families, school community, and partner organizations.
  • Accompanies program participants to office visits, as needed and according to applicable guidelines.
  • Develops and maintains effective communications with relevant stakeholders.
  • Collaborates with multi-disciplinary teams at the school and community, including COST meetings, Care Team meetings, and meetings/trainings as required by funders.
  • Maintains complete and accurate documentation, ensuring compliance of service standards and contract policies.
  • Ensure compliance with DMH billing requirements and activities provided, within CII’s documentation standards.
  • Communicates with families in our service areas, primarily Spanish-speaking populations.
  • Performs other duties and special projects as assigned

Qualifications

EDUCATION & EXPERIENCE

  • High school diploma or general education degree (GED) required. Bachelor's degree in any field preferred.
  • Community Health Worker Certification Required and/or ability to complete CHW Certificate Program within 6 months of employment.
  • Lived experience in communities for this program and/or one year of community-based direct service experience preferred.


Other Qualifications

  • Bilingual in English and Spanish required
  • Maintain annual requirement of 6 hours of continuing education courses
  • Ability to work well with others, gain consensus, and work towards a common goal.
  • Ability to work collaboratively with healthcare providers, social service agencies, and community-based organizations.
  • Ability to identify and research community resources, and initiate/develop/cultivate new professional relationships with potential members and program partners
  • Ability to work with minimal supervision.
  • Strong interpersonal skills and critical thinking abilities
  • Must have access to reliable transportation.
  • Establish and maintain effective working relationships with those contacted in the performance of required duties
  • Ability to handle confidential information.
  • Maintains patient privacy, including protecting confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
  • Sensitivity to service population’s cultural and socioeconomic characteristics.
  • Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
  • Excellent verbal and written communication skills.

Computer Skills

  • Working knowledge of computer programs in a Windows environment.

Compensation

$23.88 USD - $28.66 USD

In accordance with California law, the expected pay range for this position is included in this posting. The actual compensation will be determined based on factors permitted by law. New hires will be offered a salary at the starting point of the range to maintain internal inequity and allow opportunity for future salary growth. CII salaries are determined based on comprehensive internal and external market analysis, as well as the skills, education, and experience relevant to each position.
