Our Sanctuary Hospice team is looking for a Full-Time Registered Nurse Case Manager (RNCM) that is ready to make a meaningful difference in the lives of the patients and families we serve in and around Northern Cook and Lake Counties! This territory includes but not limitied to the cities of Waukegan, Libertyville, Lincolnshire, and Lake Forest.

- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
