Primary Accountability
Provides delivery of clinical pharmacy services, drug information and monitoring of pharmacotherapy to a specific patient population. Serves as drug expert and consultant to physicians, nursing staff and other personnel.
Description Of Primary Responsibilities
- Responsible for reviewing drug orders and checks work completed by ancillary personnel in the department.
- Routinely and consistently works in a multidisciplinary manner with a physician team/group to promote safe and cost-effective pharmaceutical care for a specialized patient population (e.g. ambulatory care clinic patients, etc.).
- Responsible for performing Medication Management Therapy for clinic patients per clinical protocols.
- Serves as a primary preceptor for pharmacy residents and students with the goals of utilizing evidence-based medicine to be an active participant in patient care, stimulating students and residents desire to learn, providing constructive feedback throughout rotations to facilitate the learning process and actively improving teaching techniques.
- Responsible for educating medical, pharmacy and nursing staff.
- Provides quality assurance review of drug utilization throughout the clinic.
- Participates in department, facility or corporate task forces and committees.
- Contributes to multidisciplinary quality improvement projects appropriate for pharmacist’s specialty area.
- Functions as a liaison to improve communications and assist with problem solving between other clinical pharmacists and health care providers.
- Performs other duties as assigned.
Professional & Technical Knowledge:
- Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in Pharmacy studies, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Pharmacy degree.
- Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
- Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
- Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
Communications Skills
- Requires the employee to effectively communicate their professional opinions and extrapolations of information they collect and synthesize/analyze.
- Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
- Duties require the preparation and execution of presentations to large groups.
