This role provides integrated behavioral health and substance use services within primary care settings.
- Conducts screenings, assessments, and crisis evaluations to tailor evidence-based interventions for diverse populations.
- Develops and modifies treatment plans, monitors progress, and ensures discharge readiness.
- Offers referrals, engages patients and families, and collaborates with medical providers to ensure coordinated care.
- Maintains documentation per policies, adheres to compliance standards, and utilizes electronic health records.
- Demonstrates cultural competence, promotes diversity, and adapts services to community norms.
- Requires a Master’s Degree, Maryland licensure, and preferably experience in outpatient mental health/substance use services, with bilingual skills being an asset.
