Position Summary
The Schenectady Prevention Program provides comprehensive clinical, case management, and family support services to Schenectady County residents. Our goal is to improve parenting skills, family relationships, child well-being and community connections. We effectively address the issues that place children and youth at risk for foster care and help families reunite after a separation or placement.
Essential Duties And Responsibilities
Position Summary
The Schenectady Prevention Program provides comprehensive clinical, case management, and family support services to Schenectady County residents. Our goal is to improve parenting skills, family relationships, child well-being and community connections. We effectively address the issues that place children and youth at risk for foster care and help families reunite after a separation or placement.
Essential Duties And Responsibilities
- Provide organizational instruction and supportive counseling with a specific focus on helping families to master transition periods.
- Provide parent coaching and support with life skills development to establish healthy routines.
- Establish positive and professional relationships with families.
- Assist Case Planner to reduce risk and increase positive attachment and communication between family members.
- Participate in the development of individual family support plan.
- Conduct supervised visitation between children in foster care and their biological parents and encourage bonding, positive interaction, recreational activities and parent skills training.
- Transport children to visitation site.
- Transport individuals and families to appointment such as medical, educational, employment, and other appointment as needed.
- Complete progress notes or other case record notes in a timely manner.
- Maintain good working relationships with DSS/Support Unit caseworkers.
- Maintain cultural sensitivity, competence, and client confidentiality.
- Model and demonstrate problem-solving and decision-making skills.
- Other duties may be assigned.
- Requires a High School Diploma or GED with 4 years of experience, OR
- Associates degree with one to two years of experience, preferred
- NYS drivers license
