What You’ll Do

Ready to change lives—and love your job while doing it? As a Student Transition Placement Specialist, you’ll be the spark that helps young adults with disabilities take their next big step into the world of work, volunteering, and independence.

  • Help people referred by the Department on Aging and the Department of Developmental Services find jobs and volunteer opportunities in the community.
  • Support program participants directly with job training, orientation, and coaching—or work with supervisors to assign the right staff to help.
  • Receive and manage referrals from the Department of Rehabilitation Services (DORS) Level Up program.
  • Fill out and turn in all required paperwork on time, including forms, progress notes, billing, and reports.
  • Meet with DORS staff and school contacts to review referrals and plan services.
  • Connect with local businesses to create job and training opportunities for students.
  • Plan and provide services to students at approved locations.
  • Use and update the curriculum as directed by your supervisor.
  • Make sure students have the technology they need to participate.
  • Work with your team and local businesses to set up job shadowing and training experiences.

Day in The Life of a Student Transition Placement Specialist:

  • Start the day by reviewing emails and confirming schedules for student job shadowing or volunteer placements.
  • Meet with school staff or agency partners to discuss new referrals and plan services.
  • Visit a local business to explore a potential placement site and build a new community partnership.
  • Support a student during their first day on the job—offering encouragement, guidance, and a quick confidence boost.
  • Return to the office to complete progress notes, update service records, and submit documentation.
  • Brainstorm creative placement ideas with your team and troubleshoot any challenges students may be facing.
  • End the day with a check-in call to a student who just completed their first week of work—and celebrate their success.

What You’ll Need

  • At least 1 year of experience helping people with disabilities or other support needs—this can be formal or informal.
  • Familiarity with agencies like DORS, the Department on Aging, or the Department of Developmental Services.
  • Basic computer skills, including Microsoft Office and data entry.
  • A valid driver’s license.
  • Good communication skills—both speaking and writing.
  • Ability to solve problems and make decisions under pressure.
  • Professionalism and the ability to keep sensitive information private.
  • Comfort working with local businesses and community groups to create job opportunities.
  • Ability to handle multiple tasks and stay organized in a busy environment.
  • Strong attention to detail when entering data and completing paperwork.

Physical, Environmental, & Mental Demands

  • Must be able to sit at a desk, stand during presentations or meetings, and walk through schools, offices, or community sites for extended periods.
  • Occasionally required to lift or carry materials such as laptops, files, or program supplies weighing up to 25 pounds.
  • Frequent use of hands and fingers for typing, writing, using a phone, and handling paperwork or small equipment.
  • Must be able to drive or travel locally to meet with students, visit job sites, or attend meetings at schools and community locations.
  • Requires clear vision and hearing to interact effectively with students, coworkers, and community partners in person or virtually.
  • Work takes place in a variety of settings, including offices, schools, community centers, and local businesses.
  • May visit job sites that differ in cleanliness, noise level, lighting, or accessibility.
  • Occasionally works outdoors during community events or site visits, which may involve walking on uneven surfaces or being exposed to weather.
  • May work in shared office spaces or open environments where interruptions and background noise are common.
  • Needs emotional strength and patience when supporting individuals with disabilities or facing personal challenges.
