The Outreach Manager is responsible for the delivery of health education to PACE eligible seniors and their families or designated representatives on valuable health care and social services via the PACE model of care. The Outreach Manager will provide direct oversight to the Community Health Coordinators and Community Health Advisors which includes but is not limited to communicating organizational needs, oversee employee’s performance and provides support as needed. This position will also be responsible for establishing and maintaining community relationships, as well as identifying and implementing organizational development.

The Outreach Manager is responsible for the delivery of health education to PACE eligible seniors and their families or designated representatives on valuable health care and social services via the PACE model of care. The Outreach Manager will provide direct oversight to the Community Health Coordinators and Community Health Advisors which includes but is not limited to communicating organizational needs, oversee employee’s performance and provides support as needed. This position will also be responsible for establishing and maintaining community relationships, as well as identifying and implementing organizational development.
Essential Job Functions
Duties include, but are not limited to:
- Supports the IIH Business Development department’s recruitment efforts and enrollment assistance for IIH’s PACE program
- Provides direct support and management to the Community Health Coordinators and Community Health Advisors.
- Completes Community Health Coordinators and Community Health Advisor training, performance evaluation and corrective action.
- Provides on-going outreach training to recruiting staff
- Participates in community health events, education activities, and PACE-sponsored events, as assigned.
- Develops meaningful, relevant relationships with community organizations including but not limited to, senior and cultural centers, churches, doctors’ offices, and/or housing communities.
- Engages in mandatory and ongoing training and skill-building activities to effectively achieve intake, enrollment, and program
- goals.
- Provides PACE referral and community health resource information to PACE-eligible seniors, their families/caregivers, and/or community members in designated service area(s).
- Conducts preliminary enrollment screening interviews to determine eligibility for PACE program services.
- Assists with the set-up of health education and PACE information booths.
- Responsible for adding and reviewing annual Community Outreach and Marketing calendar to ensure readiness and timely preparation.
- Tracks outreach activity in the Enrollment Activity Tracker
- Submits promotional material requests and reports utilization figures to the VP of Business Development and the Marketing Manager.
- Attend and participate in staff meetings, in-services, projects, and committees as assigned.
- Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.
- Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
- Be flexible in schedule of hours worked.
- May require use of personal vehicle
- Bilingual preferred
- Sales and/or customer service experience strongly preferred.
- Basic knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook).
- Ability to communicate effectively, both orally and in writing.
- Ability to quickly learn department policies, procedures, goals, and services.
- Skill: Attention to detail and accuracy.
- Ability to change priorities regularly.

- Ability to access all areas of the center throughout the workday.
- Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
- Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
- Ability to communicate verbally with an excellent comprehension of the English language.
- Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment.
Experience
- Minimum of one (1) year of sales and/or customer service experience
- Minimum of one (1) year of documented experience working with a frail or elderly population.
- Minimum of one (1) year of documented supervisory experience preferred.
- High School diploma or General Education Development (GED) required.
- Current CPR/BLS Certification
- Is medically cleared for communicable diseases and has all immunizations up-to-date before engaging in direct participant contact.
- All employees facilitating PACE marketing activities are required to pass the PACE marketing exam.
