Responsibilities
- As a Case Coordinator at DSC, you will be responsible for the coordination of a wide range of supports and services for people to whom we provide services. You will need skills in service linkage, advocacy, documentation, securing and maintain entitlements and coordination of Individual Implementation Strategy. Each day, you will be engaged in rewarding work where you will play an integral role in the success of the company and our mission to support people to live a rich and meaningful life.
- At least one year of experience working directly with individuals with a developmental disability
- RN, COTA, OTA, PTA, SLP, Audiologist, Registered Dietician, Human Services Professional with a bachelor’s degree in a human services field, including, but not limited to sociology, special education, rehabilitation counseling, psychology, recreation, art/dance/music or physical therapies.
- Knowledge of community resources and a demonstrated capacity to work in collaboration with other human service professionals.
- Valid driver’s license and reliable transportation
- 40 hour/week, 8:00 AM - 5:00 PM, Monday through Friday; One day/week remote work
- Grant-funded retention bonuses - up to 2 payments for a maximum of $1,200 per fiscal year! (depending on when in the grant year you were hired)

- All benefits start day 1 of employment
- $21.25 per hour
- 21 days of paid time off (PTO), 10 paid holidays, 1 personal holiday and 6 days of paid reserve sick leave per year
