API offers a variety of Outpatient Services designed to assist patients in functioning at the highest level. API provides Partial Hospitalization services and Intensive Outpatient services. These programs emphasize symptom management, community integration, and prevention of hospitalization or re-hospitalization. Value-added services enhance the patient's treatment experience.
Alvarado Parkway Institute is currently seeking a qualified individual to join our team as a Health Information Services Clerk. The Health Information Services Clerk performs various daily job task assignments in a timely, accurate and detail oriented manner. HIS Clerks assist in the coordination of all the technical aspects of the day to day operations of the department and seeks to organize and achieve quality performance, efficiency, effectiveness towards the achievement of goals and objectives while preserving the confidentiality of privileged information. Coordinate and maintains, with the cooperation of other disciplines, written records of each patient admitted for treatment and for attainment for all requirements established by licensing, certifying, and accrediting agencies related to the medical records.

Requirements
HIS Clerk Responsibilities:
- Assists physicians and staff with medical records questions, problems, and requests. Provides outpatient medical record services willingly as assigned
- Answers department phone promptly and is polite and helpful with their requests; takes and routes messages as necessary; refers persons to the appropriate staff member for questions or requests for which cannot be fulfilled.
- Is pleasant when people enter Medical Records Department and helpful in directing them
- When notified of staff coming to medical records to correct deficiencies takes the initiative to pull the charts and/or have them ready for the staff member to complete
- Assembles D/C charts per D/C chart order
- Seeks out work that needs to be done by helping co-workers and exhibiting flexibility (i.e., loose filing, storage and retrieval of charts, photocopying, faxing, mailing, charts assembly, maintaining files, etc.)
- Recognizes, establishes, and deals with priorities/task sequences promptly, displaying good time management, productive results, and pro-active outcomes
- Demonstrates reliability and punctuality in attendance; abides by facility policy and procedure regarding paid time-off
- Functions appropriately and productively, displaying professionalism while working, giving attention to detail
- Ability to establish and maintain ongoing communications with and is responsive to suggestions from co-workers, Supervisor, those in authority positions, and other departments
- Demonstrates good judgment by carefully following written and verbal instructions, formats, and promptly communicating any problems/concerns to Supervisor
- Demonstrates ability to handle personal and professional frustrations appropriately in order to maintain smooth workflow and department productivity
- Other duties as assigned.
- Typing skills required with the ability to operate a computer system
- Able to use various resource materials including, but not limited to copy machine, fax machine, etc.
- Able to think, work and function independently and perform assignments productively

- Able to establish, maintain, & communicate well with staff & providers, expressing ideas effectively
- Possesses good organizational, leadership, problem-solving, communication, and motivational skills
- High School Education or Equivalent.
- Graduate of a Certificate Program for Medical Records.
- Experience in the Mental Health field
While performing the duties of this job, this position is frequently required to do the following:
- Use standard office equipment and access, input, and retrieve information from a computer.
- Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time.
- Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population.

In compliance with API’s policies and guidelines, all API employees are required to adhere to the following:
Background/Reference Check & Licensure/Certification
- Completion of a satisfactory background and reference checks.
- Employee signed employment non-compete/confidentiality agreement
- Individuals whose position may require licensure, registration or certification must maintain such as current and valid. It is the employee's responsibility to send updated copies to Human Resources before the expiration date. It is the employee's responsibility to notify Human Resources of any changes or proposed changes in status to licensure, registration, or certification. Failure to do so could result in employee suspension.
- Provide a copy of COVID-19 Vaccination OR have an Exemption Request (Deferred Pregnancy, Medical, or Religious) approved by the API HR Director; and those who have an approved Exemption must undergo weekly COVID-19 testing and observe all other infection control requirements, including face coverings.
- Annual Flu vaccination is required as a condition of hire and ongoing employment. Only those who are allergic to the vaccine or meet contraindications as indicated on the current CDC Vaccine Information Statement shall be exempt from this condition of hire and ongoing employment but are required to wear a mask while inside an API owned or operated building or vehicle during influenza season, Employees must provide physician documentation to support any reported allergy or contraindication. Flu season typically runs between November and March.
- Pass drug and alcohol test performed by Concentra.
- Clear TB skin test or Chest X-Ray at Concentra.
- Complete a satisfactory physical examination at Concentra designed solely to determine your ability to perform the duties of the position being offered.
