Schedule: Full-Time Days – 11a-7p/12p-8p (Flexible Hours)

- Conduct in-home behavioral health assessments and develop individualized care plans.
- Provide compassionate, one-on-one patient care focused on both physical and mental well-being.
- Educate patients and families on health management and coping strategies.
- Communicate patient progress and any concerns with physicians and the clinical team.
- Adjust care plans based on patient needs and treatment effectiveness.
- Mentor and supervise behavioral health aides and LPNs.
- Active, unrestricted RN license in the state.
- Minimum of one year of nursing experience in a clinical or behavioral health setting.
- Ability to travel within the assigned coverage area (50% travel required).
- Reliable transportation with a valid driver’s license and current auto insurance.
- Associate or Bachelor’s degree in Nursing (preferred).
- Prior home health or hospice experience is a plus.
