Responsibilities
Your Role: The Heart of Transition
As a Transitional Care Liaison, You'll Be The Vital Link Between Patients, Families, Hospitals, And Our Centers. Your Key Responsibilities Will Include:
- Conducting comprehensive clinical assessments to determine patient appropriateness for our centers, collaborating closely with interdisciplinary care teams to ensure a smooth and safe transition.
- Partnering with center teams to achieve and exceed key account management and admission goals, contributing directly to the success of our centers.
- Articulating the unique features, benefits, and positive impact of our centers using effective sales strategies and the Diversicare Sales Steps.
- Communicating professionally and effectively through clear written documentation and verbal interactions.
- Serving as a champion for quality care and a dedicated advocate for outstanding customer experience.
What We're Looking For:
- Registered Nurse (RN) preferred; Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), Physical Therapist, Occupational Therapist, Speech Therapist, or Respiratory Therapist with a current license in good standing.
- 1-2 years of experience in skilled nursing or long-term care.
- 1-2 years of experience in sales, marketing, or admissions is highly desirable.
- Established relationships with hospital and physician customers are a plus.
- Demonstrated strong medical assessment skills and commitment to HIPAA compliance.
