Job Summary

Golden Modesto Care Center in Modesto, California is seeking a Case Manager to join our in-house team. The Case Manager is responsible for coordinating care, discharge planning, and acting as a liaison between residents, families, and healthcare providers to ensure the highest quality of care.

Essential Functions:

• Coordinates and oversees the resident care planning process from admission through discharge.
• Collaborates with residents, families, nursing staff, social services, therapy, and physicians to ensure individualized, effective care plans.
• Supports residents and families with discharge planning, home care referrals, or placement in alternate levels of care.
• Monitors residents' progress and updates care plans accordingly.
• Ensures compliance with Federal and State regulations regarding resident care and documentation.
• Advocates for residents’ needs and rights.
• Maintains accurate and timely documentation in accordance with facility and regulatory standards.
• Participates in interdisciplinary team meetings and quality improvement initiatives.
• Communicates effectively with all levels of staff, residents, and families.
  
  
  

Education, Licensure, & Experience Requirements:

• Must possess either:
• A current and valid LVN license in good standing in the state of California, or
• A degree in Social Services or related field.
• Prior experience in case management, discharge planning, or social services in a skilled nursing or healthcare setting preferred.
  
  

Knowledge, Skills and Abilities:

• Strong interpersonal, organizational, and communication skills.
• Ability to work effectively in a multidisciplinary team.
• Knowledge of Medicare/Medicaid regulations and community resources.
• Familiarity with electronic medical records and documentation standards.
