We are currently seeking a Remote HCBS Service Coordinator to support individuals receiving Home and Community-Based Services (HCBS). In this role, you will help clients access the support they need to live safely and independently in their communities—all while working from the comfort of your home.

Key Responsibilities

• Coordinate and monitor HCBS services for a remote caseload of clients
• Develop and update person-centered service plans
• Collaborate with clients, families, providers, and interdisciplinary teams via phone or video
• Ensure compliance with New York State guidelines and documentation standards
• Advocate for client needs and connect them with appropriate community resources
  
  

Qualifications

• Bachelor’s degree in social work, psychology, human services, or a related field
• Experience with behavioral health populations strongly preferred
• Excellent communication, time management, and documentation skills
• Familiarity with HCBS programs and case management is highly preferred
