Location: Brooklyn, NY
Pay Rate: $26-30/hour
Key Responsibilities
- Schedule and conduct in-person assessments in home and community settings while following a hybrid work model.
- Travel within the NYC Region to meet with individuals, families, and caregivers.
- Use a person-centered interview approach to assess individuals' needs and strengths.
- Ensure accuracy and completeness of assessments in compliance with state guidelines.
- Interview caregivers, providers, and other key stakeholders to gather necessary information.
- Utilize multiple data sources, including records review, for assessment coding.
- Enter data into assessment software and meet documentation deadlines.
Education
- Bachelor’s degree in a human services field
- Minimum one (1) year of direct experience working with adults with intellectual and/or developmental disabilities (IDD)
- Strong analytical and problem-solving skills
- Ability to work independently while meeting deadlines
- Excellent written, verbal communication, and presentation skills
- Proficiency in Microsoft Word, email, and web-based applications
- Bilingual (English/Spanish) – Preferred
- Must possess a valid NYS Driver’s License or NYS ID card
- Must be willing to travel throughout assigned location
